If I had to bet, it came down to one of two things: 

1. At their core, they weren't inwardly sound
2. Or, they weren't other's focused

Today I want to remind you to ask yourself, your trusted advisors, and your direct reports where you need to grow in your leadership. It might help to have those people pick one thing from this list.
